To learn more, click here .For our Mission, click here .POSITION SUMMARY:The ICC, using the principals and guidelines set forth by the Children's Behavioral Health Initiative, provides coordination of care to children with an emotional or behavioral health need on Mass Health and to their families. The ICC will also provide Care Coordination to families working with the Department of Children and Families to provide Family Support and Stabilization Services. The children and families that work with the Walden Community Services Program must have a deaf member.ICC Functions and Responsibilities:Works with youth and family to determine medical necessity eligibility by completing required assessment tools.Works with the family to identify professional and natural supports.Develops Care Plan Team with the family and the youth.Works from a strength based and family centered perspective.Works as a liaison with collateral agencies, schools, doctors etc.Please note this job description is not designed to cover or contain a comprehensive listing of activities, duties, or responsibilities that are required of the employee for this job. Duties, responsibilities, and activities may change at any time with or without notice.RequirementsA Master's Degree in Social Work, Counseling or other clinical related field. A BA in a human service field plus five years experience will be consideredAbility to be licensed at the Independent Practice Level in your discipline (LMHC, LICSW, etc.) preferredA minimum of conversational fluency in American Sign Language required, with native, or near-native ASL fluency preferredBicultural competency and effective interpersonal and communication skills will be necessary to work collaboratively with the teamTime flexibility with some availability during evening hours requiredRoutine travel and in-home visits requiredStrong Microsoft Office skills and proficient with Mac/Apple computers This position open until filled.
