Job Description Job Description COMMUNITY BRIDGES IS GROWING - JOIN OUR TEAM! $1,000 Sign-On Bonus Community Bridges is seeking a full-time Behavioral Home Direct Support Professional (DSP) in Boscawen, NH . Founded in 1982, Community Bridges is a nonprofit organization dedicated to advancing the integration, growth, and interdependence of people with developmental disabilities within their communities. Serving more than 1,100 individuals and families across the Greater Capital Area, we work to remove barriers, create opportunities, and foster meaningful community connections. Position Overview The Behavioral Home Direct Support Professional serves as a community connector , supporting individuals through community mapping and intentional engagement to promote social inclusion. This role focuses on building relationships, identifying strengths, and expanding opportunities that support each individual's growth, safety, and integration within their community. DSPs promote abilities-not disabilities by helping individuals build meaningful connections with family, friends, and community members while encouraging independence and personal development. Qualifications Education High School Diploma or GED required Experience Experience in human services or working with individuals with disabilities preferred Additional Requirements Reliable transportation Valid driver's license Proof of automobile insurance Compensation & Benefits $1,000 Sign-On Bonus $500 paid at hire $500 paid after 6 months Sign-on bonus applies to new hires only. Rehires and transfers are not eligible. Paid Time Off (PTO) 22 days annually for full-time employees (40 hours/week) PTO increases with longevity No waiting period to begin using PTO Insurance & Retirement Medical, Dental, Vision 403(b) with 1% company match Life Insurance Employer-paid Short-Term Disability Mileage Reimbursement Reimbursement for approved mileage driven while on the clock Paid Training Training time paid at your regular hourly rate Available Schedules Full-Time – 12-Hour Shifts: 7:00 a.m.–7:00 p.m. 7:00 p.m.–7:00 a.m. Key Responsibilities Support individuals with planned activities and community involvement Develop relationship maps to identify social inclusion opportunities Provide continuous supervision and intervene as needed to maintain safety Assist individuals in building and expanding social networks Promote self-esteem through meaningful community participation Provide individualized supports, including personal care and skill development Assist with hygiene supports as needed Participate in Person-Centered Planning and goal implementation Complete required documentation, including data collection and progress notes Assist with orientation and training of new staff Administer medications in accordance with He-M 1201, as applicable Report safety, health, or human rights concerns immediately Ensure compliance with state regulations and agency policies Perform other duties as assigned Required Skills Ability to remain calm and respond effectively in high-risk or escalated situations Strong judgment and decision-making skills Ability to maintain professional boundaries while building rapport Strong verbal and written communication skills Ability to meet documentation deadlines Ability to work independently and as part of a team Ability to support daily living and functional skill development Ability to utilize community resources Basic computer skills, including email and word processing Positive, professional representation of Community Bridges Work Environment & Physical Demands Work Environment Primarily indoor residential setting Occasional noise and high-stimulation periods Staff expected to provide support during stressful situations Physical Demands Ability to lift up to 50 lbs Frequent walking and standing; occasional stair climbing Ability to push wheelchairs and operate lifts Ability to follow behavioral intervention techniques Travel Requirements Access to a personal vehicle during scheduled work hours Travel with individuals using personal or agency vehicles Valid NH driver's license and safe driving record Adequate automobile liability insurance Communities Served Concord (Headquarters), Allenstown, Andover, Boscawen, Bow, Bradford, Canterbury, Chichester, Danbury, Deering, Dunbarton, Epsom, Franklin, Henniker, Hill, Hillsborough, Hopkinton, Loudon, Newbury, New London, Northfield, Pembroke, Pittsfield, Salisbury, Sutton, Warner, Weare, Webster, Wilmot, Windsor
